Phillip Swann of tvpredictions.com offers his list of the 10 stars whose true looks make you stagger backward in disbelief when you finally see them as they are, in realer–than–real high definition.
In order, from biggest [unpleasant] surprise (Brad Pitt) down, they are:

3) Cameron Diaz
4) Maria Sharapova
5) Madonna
6) Renee Zellweger
7) Kurt Russell
8) Ray Liotta
9) Frankie Muniz
10) Tommy Lee Jones

Brad Pitt's bad skin is pretty old news. It's pretty clear even on non-HDTV in his breakout (haha) movie Thelma and Louise.

at least the first three people on the list have REALLY bad skin and you could sometimes see it on Friends underneath the makeup on J.A.'s face. she and BP went to skin clinic together to have their skin worked on...to little avail...Same with Cameron Diaz...she's really scarred...pockmarked.
And Madonna, although she's my favorite in the world, she's kind of wrinkly and saggy (face)...
