When I see little bitty toothpicky, dental-flossy things like this, I always wonder if they don't get swallowed sometimes. Well, rarely...but still. When I was fourteen I had an ink pen with a complicated clicker mechanism that had a 2-inch piece of plastic that could, unbeknownst to me at the time, come detached from the pen and fly through the air. And one day, while absent-mindedly clicking it, that piece flew right down my throat. So I know it's possible to swallow something that size, and most people probably aren't going to think about getting a death grip on their mini-dental-picky tool.

I never 'recovered' (💩) the flying plastic piece, but I assume it migrated to the outside easily, since I never had any problems.
